Residence (Foreign Police)

Applications for granting, renewing and permanent residence, proof of accommodation, booking and electronic filing.

Application for temporary residence

Issued by: Ministry of Interior – Foreign Police

Official application for a first residence permit: employment, business, study, family reunification. The form (several languages) is on the Ministry page under “Pobyt cudzinca”; fill it in block letters with data exactly as in your passport. Filed in person at the Foreign Police department or at an embassy together with the attachments.

Form for extending a valid permit (PDF from the Ministry website, English version). Filled in the same way as the first application; the current card number and address must match the police records. Filed before the permit expires at the Foreign Police department or electronically via portal.minv.sk.

Permanent residence for 5 years, for an unlimited period or EU long-term residence – each has its own list of attachments. The form is issued by the Foreign Police; the IOM Migration Information Centre page explains the types and attachments. Check which type applies to you before filing.

Accommodation declaration (owner’s consent)

Issued by: Ministry of Interior – Foreign Police

Every residence application needs proof of accommodation: a confirmation from a hotel or hostel, or a sworn declaration by the property owner or an authorised user. The owner signs the declaration with a certified signature; property data must match the land register. The owner can also give consent electronically via portal.minv.sk. Accepted forms are listed in the Ministry FAQ.

Online appointment at the Foreign Police

Issued by: Ministry of Interior – Foreign Police

Applications in person are accepted only after booking through the Ministry system. Choose the department by your address, the type of matter and a free slot; a confirmation with a code arrives by e-mail and you show it at the entrance. Without a booking the application is usually not accepted.

Issued by: Ministry of Interior – e-services

Portal for electronic filings: residence renewal, application for a residence card or alien’s passport, owner’s consent to accommodation, registration of accommodation facilities. Requires an electronic ID (eID with activated chip or another recognised means) and a qualified signature for attachments. Electronic filing does not replace the in-person visit for biometrics.

Temporary protection (dočasné útočisko) – official information

Issued by: Government of Slovakia and Ministry of Interior (ua.gov.sk)

Official portal for Ukrainians: how to apply for temporary protection, where, which documents to bring, how to extend or change data. Information in Ukrainian, Slovak and English. The application is filed in person at the Foreign Police or a large-capacity centre; the form is completed on site.

Trade licence notification – natural person

Issued by: Ministry of Interior – trade licensing department of the district office

Official form for registering a free, craft or regulated trade. It states personal data, the business address, the business activities exactly as in the official list and a responsible representative where required. Filed at the trade licensing department (single contact point) or electronically via slovensko.sk.

Single contact point (JKM) for starting a business

Issued by: Ministry of Interior – trade licensing department of the district office

When you register a trade, the JKM also arranges tax registration, notifies the health insurer and makes entries into registers. The page lists offices by region and the range of services. Bring your passport, residence document, criminal record extract and a document for the business address.

Trade register e-services

Issued by: Ministry of Interior – trade licensing department of the district office

Electronic trade registration, notification of changes, suspension or termination and register extracts via slovensko.sk. Requires an electronic mailbox and a qualified signature. Electronic filing is usually cheaper and needs no office visit.

Income tax registration for individuals (DIČ)

Issued by: Financial Administration of the Slovak Republic

After a trade licence is issued, the tax office assigns a DIČ. If the JKM did not handle it, the registration form is filed electronically through the Financial Administration portal. The page explains who registers, when, and what to report later. Entrepreneurs communicate with the tax office electronically only.

What the employer files with the labour office when employing a foreigner.

Information card – employer notifies start and end of a foreigner’s employment

Issued by: Office of Labour, Social Affairs and Family (ÚPSVaR)

Filed by the employer, not the employee: it notifies the labour office of the start and end of a foreigner’s employment. The form is electronic and covers employer, employee, residence card and contract details. Employees should check that the card was filed – it matters for renewal.

Confirmation that a vacancy can be filled (for a work-based permit)

Issued by: Office of Labour, Social Affairs and Family (ÚPSVaR)

The employer first reports the vacancy to the labour office; the Foreign Police then requests the confirmation for the residence application for employment. The slovensko.sk page describes the electronic service for employers. The applicant files nothing but should know the vacancy was reported.

Health insurance

Applications to the three public health insurers and who qualifies for public insurance.

Health insurance application – VšZP

Issued by: Všeobecná zdravotná poisťovňa (state health insurer)

Online application for public health insurance. A foreigner with residence and a job or business in Slovakia picks one of the three insurers; you fill in personal data, residence card number, address and the basis of insurance (employee, self-employed, self-payer). The insurer issues a card you show at the doctor.

Online application to Dôvera – the same public insurance conditions as the others. You need residence card data and the basis of insurance; sign electronically or at a branch. Changing insurer is possible once a year in a set period.

Online application to Union. Choose who you apply for (yourself, a child), enter residence card data, address and the basis of insurance; confirmation arrives by e-mail. Public insurance at all three insurers covers the same scope of care.

Slovak criminal record extract, apostille, driving licence, recognition of education.

Application for a Slovak criminal record extract

Issued by: General Prosecutor’s Office – Criminal Register

The extract is needed for a trade licence, some residence types and employment. Apply in person at a post office (IOMO) or the prosecutor’s office with your passport; foreigners also state parents’ names and residence data. The page has forms and explanations; the extract is usually issued on the spot.

Criminal record extract online (eID)

Issued by: General Prosecutor’s Office – Criminal Register

Electronic application for those with an eID with activated chip (a residence card with chip also works after activation). The extract arrives as a PDF in the electronic mailbox and is valid for electronic filings; for paper filings you need a certified conversion or a paper extract.

Apostille and superlegalisation of foreign documents

Issued by: Ministry of Foreign and European Affairs of the Slovak Republic

Documents from Ukraine (civil status records, diploma, criminal record) must be certified with an apostille in the issuing country and then officially translated for Slovak authorities. For states outside the Hague Convention, superlegalisation at a Slovak embassy applies. The Ministry page explains which procedure applies to which country.

Official page on when a foreign licence is recognised and when it must be exchanged for a Slovak one. Apply in person at the traffic inspectorate for your place of residence with your residence card, the original licence and its official translation; the form is completed on site.

The navigator tells you whether your diploma or certificate needs recognition for work, study or a regulated profession and who handles it (ministry, university, district office). Then an electronic or paper application follows with the apostilled and translated document.
