1.What we need from you
You provide
- Child’s passport
- Birth certificate with apostille and certified translation with us
- Consent of the other parent if the child is not in their care notarised, apostille + translation
- Parent’s Slovak residence document
- Criminal record extract (child aged 14+ only) with us
- Proof of housing for at least 6 months: notarised owner declaration, notarised lease or accommodation provider confirmation signed by all owners; signatures notarised; not older than 90 days; we check it before filing
- Health insurance in Slovakia with us take out within 3 working days of receiving the card, submit within 30 days; not attached for employment and study; we help choose an insurer
- Power of attorney for migrant.sk with us our template; signature certified (or signed at the foreign police)
We arrange
- No state fee (under 18); card €10
Where clients most often fail: housing and finances
- Housing: you need a notarised declaration by ALL owners, or a notarised lease in your name. A plain lease without certified signatures is not enough. At least 6 months.
- Finances: a bank confirmation of the balance on an account in your name. The amount is 12× the subsistence minimum (from 1 Jul 2026 €3,542.64 in total); for business an extra separate business account. Not older than 90 days.
- Every document issued abroad needs an apostille (or superlegalisation) and a certified Slovak translation. Translate after the apostille.
- No document may be older than 90 days on the filing day (except marriage and birth certificates).
